LICENSE APPLICATION
Please fill out the form completely. You will then receive a combination invoice / License for the reported usage. License applications (one per production) must be completely filled out and submitted to us prior to broadcast or distribution of the production. Failure to report usage within 30 days of synchronization may result in penalties which will substantially increase the license fee.
